I work together with you in the following way. 

Your first appointment will be focused on me getting to know you, your health, your food choices, and your likes and dislikes. Your second appointment is where you will receive your personalized eating plan. No eating plan is the same thus I do need time to put this together for you. This usually takes 5 – 7 working days. 

My goal is to educate you about nutrition in such a way that I can empower you to make good choices and not only rely on rigid meal plans. The aim of our relationship is to help you to maintain a long-term sustainable healthy eating pattern that will become a lifestyle.

**Online Meal Plan includes a questionnaire answered by the client and their meal plan will be emailed to them within 7 working days based on the answers they give on the questionnaire. 

DOCUMENTS & CONSENT FORMS 

You will be required to fill in and sign ALL of the below 5 documents before (or on the day of your initial appointment).  

These forms include the following and are attached to this email. 

  1. Consent Form indicating consent to Nutrition Therapy 
  2. POPIA Consent Form indicating consent to the storing of your information
  3. Patient Information Form for your file with your personal information, medical aid details,  and declaration of the person responsible for your account. 
  4. Food List indicating foods that are disliked or not eaten by the patient. 
  5. Signed Practice Policy whereby you have agreed that you have read through and understand the practice policy and terms and conditions. 

Please note that the online submission of your details goes to the accounts team and is required for your invoicing and billing. Should you not complete this, the team will be unable to invoice you correctly for submission to medical aid. Please ensure you have completed this online form prior to your initial appointment. (You would have received an online link for this upon confirmation of your initial assessment).

BILLING & INVOICES 

The practice payment terms are cash, card, or EFT upfront after the consult. Please note card payments come with an additional 3% handling fee. 

Appointments are time-based and thus billed accordingly – please note that should we go consistently be going over the time allocated – this will be billed for. 

Any additional motivation or referral letters required will be billed unless this has otherwise been discussed and agreed upon by both parties. 

Please do NOT reply to your booking reminder or invoice reminder. This is a system-generated email and the replies do not come to my email. I will not get your email if you reply to a booking reminder or invoice.

MEDICAL AID SUBMISSIONS 

The practice is NOT contracted into any medical aid groups and does not submit on behalf of the client. It is the responsibility of the client to submit the relevant documentation to their medical aid group for potential reimbursement. 

All consults can be submitted to medical aid. On receipt of payment, you will be issued an invoice reflecting the relevant ICD10 codes. Reimbursement will depend on your plan, available savings,  chronic disease benefit, and/or any other benefits. Please note the practice is not responsible for submitting your invoices nor will it be liable for any shortfalls. 

Please do not submit UNPAID invoices to medical aid – this could result in a double payment where you and medical aid make payment to the practice. The accounts team will then need to rectify this which takes time. Once you have made a payment, a PAID invoice will be issued for you to send to medical aid for them to reimburse you. The accounts team will not be transferring  double payments back to the client but your account will be credited for the amount for future sessions.

CONSULTATIONS: 

Upon booking the initial consultation or Vitality nutritional assessment, you will be emailed relevant forms (see above point under documents and consent forms) to complete prior to your consultation. Although it is not compulsory to complete these forms before, it will be a valuable time saver during the consultation process. 

Initial consultation: This is a 75-90 minute consultation that will include a thorough assessment of your lifestyle, medical history, current medications & vitamins, physical activity, dietary habits, and current dietary intake, as well as your body composition (i.e. weight, body fat %, muscle mass, etc).  We will also discuss any family history of different conditions, assess your weight history, and previous diets or eating plans, and set your goals. The relevant blood tests that may be required will be discussed. (If blood tests are required, the dietitian will give you a blood test form with the appropriate tests marked). This will be followed by an individualized nutrition education that assists individuals to understand the following key concepts of nutrition. 

Second consultation: This consultation is 45 minutes long and will be scheduled within a week after the initial consultation. The dietitian will develop an individualized meal plan tailored to your lifestyle, health, weight, nutrition and health goals in the form of a 7-day cycle menu, vitamin and other supplementation advice will be given, and appropriate recipes and shopping lists will be presented to the individual at this consultation. The meal plans are specially designed to take into account individual lifestyle and dietary requirements so that the plan is manageable. Feedback on your lab results will also be done if you have gone for the relevant blood tests. 

Follow-up consultations: These consultations are recommended to help improve the implementation of the lifestyle and dietary changes recommended by the dietitian. During these consultations, individuals will be weighed, food records will be evaluated and discussed, and new goals will be set. Research has found that follow-up appointments are essential to keep individuals motivated. Furthermore, follow-up monitoring and support assist individuals in finding solutions to barriers to compliance that ultimately contribute to greater success. The frequency of follow-up appointments is scheduled to suit the client’s preference. The duration of all follow-up appointments is 30 minutes. Regular follow-ups are extremely beneficial to assist you with staying on track. Follow-ups are usually scheduled every 1 to 2 weeks and are the ideal opportunity to review your progress and provide ongoing support and monitoring. Incorporate program changes as needed. Receive further guidance & motivation. Clarify any questions or concerns that may have arisen. Review your goals and monitor your progress. Establish new goals and achievements. Discuss new ideas,  questions, or concerns, and add/build new concepts, while providing continued support and motivation. This is also where we would adjust your meal plan, review portion sizes, and add additional recipes. Any changes to your meal plan after a follow-up will be sent within 3 working days. *Please note: Follow-up appointments made more than 6 months since the last consultation will be billed for 45 minutes

InBody Composition Analysis: Weight is a sum of many different factors and hence we use a  more detailed report that we get from the bio-electrical impedance InBody 120 scale. An InBody composition assessment is a full body analysis that includes the measurement of your height,  weight, BMI (body mass index – (kg/m²), and Basal Metabolic Rate (kcal). It includes a body fat 

analysis (total fat mass (kg), body fat percentage (%), fat distribution (kg), and visceral fat level reading. It also includes a muscle analysis (skeletal muscle mass (kg), lean muscle distribution (kg), and total body water reading. You will receive a detailed report with personal recommendations.

Discovery Vitality Nutritional Assessment*: The Vitality nutrition assessment can be had by any vitality member once a year. These one-on-one assessments usually take 20-30 minutes and your lifestyle and nutrition history are assessed, anthropometry is measured, and healthy lifestyle goals and guidelines are established. 1000 Vitality points are awarded for the assessment *Meal plans 

are not included in this assessment, however, should you wish to have a meal plan, please book a full appointment and you will still get the same amount of points. 

RESULTS AND GOALS 

In order for you to achieve results and reach your goals, YOU will be required to put in the hard work and discipline. I am here to educate you, provide you with the tools, and give you advice, structure,  and accountability. Your success depends on YOU doing the work which includes the following:  following guidelines and plans, booking and attending follow-up appointments, being open and honest in our sessions, and asking for help. I do not possess a magic wand and it is important for  you to remember that change & results take PATIENCE, DISCIPLINE, AND TIME. You will not see the results you want if you are not willing to focus on those three key elements. 

BOUNDARIES 

The relationship between the client and the healthcare provider is a professional one that requires boundaries and respect from both parties to work. 

Emails & WhatsApp messages will only be responded to during the week. Please note that there will be no response over a weekend unless this has been discussed & agreed upon. Please remember my day is spent with other clients, which means I am unable to respond to  

calls, emails, or WhatsApp messages during this time. I will get back to you within 24-48  hours of you sending an email or a message. 

I insist on professional respect at all times. This means using respectful language in our correspondence, being on time for your session, letting me know if you can’t make your session in advance, and/or letting me know that you are going to be a few minutes late. This goes both ways and I will do the same with you.  

Any further advice or a significant number of questions will not be answered over email or  WhatsApp due to legal and ethical reasons. Please ensure you write down questions you have in-between our sessions and bring these to your follow-up appointment to discuss.
